There was never a factory chrome grill on a '66. Painted steel or anodized aluminum.

Its my understanding the Custom cab trucks were top of the line back in those times. And received the bright work inside and out...
Otherwise it was painted grill and front bumper, rear bumpers were an option regardless of trim level.
Mirrors, rear bumpers, hazard lights, spot lights and such luxurys were a dealership add on, none were installed at the factory, but may have been packed and shipped with the truck to be installed by the dealer.
But, chrome front bumper, two-tone paint, an extra coat of paint, and almost any option could be ordered separately on any level.

IE in early 61 if one ordered a custom cab...
A chrome grill, white bumper, custom cab emblems on doors, two tone dash and roof with chrome gauge cluster and ( "no small chrome strip across the dash separating the two tone, that came late 61early 62" ) bright work around the front and rear glass were standard. In later years that list grew as to what was standard for a trim level and what was an option.

This is by no means an exhaustive list. If you want that, you would want a sales mans fact sheet for each year. This would should options and extras for each trim level and differences for each year... I see them come up on eBay every so often...

My brothers 66 F-100 was a Custom Cab and did have a chrome bumper also. The 65 chrome steel I had was a wrecking yard piece. Mine had been out for 26 years and my brothers 66 was about 27 or 28 about the time he got it. His appeared to be an undisturbed truck. Traded mine for a set of heads and kept the aluminum grille. His truck sold whole. Factory or not the 65 was a nice straight heavy grille.

So best I can decipher from the salesman's fact book... Chrome bumper would have been on a Custom cab with the Deluxe appearance package added.... The only non available option you have mentioned is a "Steel" Chrome grill... That would be non Ford, but easy to have done back in the day.
